Permission to Release Form (FERPA) – Due to Federal Regulations the Financial Aid Office is unable to release student financial aid information to third parties (this includes parents & spouses). To authorize PAU/PGSP to discuss your financial aid information with a specific person or agency please download and complete the attached form. A separate form must be completed and turned in to the Financial Aid Office for each person/agency. Download Here
Low income statement- Additional income verification may be needed for students with low income information provided on the FAFSA. The Financial Aid Office will notify you if additional form is needed. Download Here
